The person needs more pillows to sit upright to sleep.

Alkaline Water

Colonoscopies Should Last at Least 6 Minutes

(HealthDay News) -- Doctors who perform colonoscopies for early detection of colorectal cancer get better results if they spend at least six minutes looking for abnormal growths, a new study shows.

The study recorded the experience of 12 experienced gastroenterologists who did 7,882 colonoscopies, 2,053 of them for patients who'd never had a colonoscopy. In 501 of those first-time -- or screening -- exams, the amount of time spent was less than six minutes.

Those exams were the least productive. In all, cancerous or precancerous growths were detected in 23.5 percent of those patients examined. But such growths were detected in only 11.8 percent of exams where the period of examination -- called withdrawal time -- was six minutes or less; growths were detected in 28.3 percent of exams with a withdrawal time of more than six minutes.

"We think our data suggest that if the goal of colonoscopy is to detect and remove polyps, spending slightly more time in the procedure seems to translate into a better outcome for patients," said study co-author Dr. Robert L. Barclay, a partner in Illinois-based Rockford Gastroenterology Associates.

The findings are published in the Dec. 14 issue of the New England Journal of Medicine.

"This basically reinforces and provides evidence for the quality recommendations that have already been made," said Dr. Durado Brooks, director of colorectal cancer for the American Cancer Society.

Withdrawal time is "a very important measure of the quality of colonoscopy," Brooks said, and the U.S. Multisociety Task Force on Colorectal Cancer has recommended a minimum withdrawal time of six minutes.

"This study clearly shows that six minutes is a critical point, and those patients who had less had a strikingly low rate of adenoma [growth] detection, and there is at least a suspicion that the cancer detection rates would be lower," he said.

Brooks applauded the report. "I'm not sure that there has been a study as well done as this one showing a marked difference between withdrawal rates less than six minutes and more than six minutes," he said.

Barclay said the members of his medical group decided to do the study because "we do a lot of screening in our practice, with an increasing volume causing us to spend a disproportionate amount of time on it, and we wanted to make sure we were doing a decent job."

The results of the study have led to a change in the way colonoscopy is done by the group, he said.

"We've reviewed these results, and they were so impressive that we now take an extended minimum amount of time," Barclay said. "We have measured detection rates for a withdrawal time of eight minutes, and we found that with that simple step, there was a dramatic increase in polyp detection among all our physicians."

Medicare's Hospital Ratings System May Be Flawed

(HealthDay News) -- Medicare's standard hospital-performance measures may not accurately reflect real-life patient outcomes, U.S. researchers report.

They found little difference in death rates for three common conditions -- heart attack, heart failure and pneumonia -- at hospitals with low and high Medicare performance measures. The study, by researchers from the University of Pennsylvania and the Philadelphia Veterans Affairs Medical Center, analyzed ratings for more than 3,600 acute care hospitals in the United States.

The study included Centers for Medicare and Medicaid Services' (CMS) data on hospital care between Jan. 1, 2004, and Dec. 31, 2004. The researchers found that, across all heart attack performance measures, the absolute reduction in risk-adjusted death rates between hospitals in the 25th percentile and those in the 75th percentile was 0.005 for inpatient death, 0.006 for 30-day death, and 0.012 for death at one year.

For heart-failure performance measures, the absolute reduction in deaths was very small, ranging from 0.001 for inpatient death to 0.002 for death at one year, and for pneumonia it ranged from 0.001 for 30-day death to 0.005 for inpatient death.

The findings are published in the Dec. 13 issue of the Journal of the American Medical Association.

"Our study suggests that in the case of hospital performance, the CMS's current set of performance measures are not tightly linked to patient outcomes," the study authors wrote.

"These findings should not undermine current efforts to improve health care quality through performance measurement and reporting. However, attention should be focused on finding measures of health care quality that are more tightly linked to patient outcomes. Only then will performance measurement live up to expectations for improving health care," they concluded.

FDA Panel Urges Changes to Antidepressant Labeling

(HealthDay News) -- A U.S. Food and Drug Administration advisory panel recommended Wednesday that "black-box" warnings on widely used antidepressants be changed, but the exact wording on the warning is still up in the air.

The issue at hand was whether current warnings on an increased risk for suicidal thoughts and behaviors in children and adolescents should be extended to young adults, or even older patients.

All eight members on the panel advocated some kind of change in labeling after a day-long meeting, according to an FDA press briefing held late Wednesday. But in the end, six members voted to extend the current suicide warning beyond children, but no exact age cut-off was specified.

"The vote that we took was on two issues: No. 1 was, 'Is there a need to change the current labeling?' And there was a unanimous vote in favor. And issue No. 2 was, 'Should that change be made within the black box?' And once again we voted in favor," said panel member Dr. Daniel Pine, chief of child and adolescent research at the National Institute of Mental Health.

"There was a great deal of discussion -- but no presentation and no vote -- of the specific wording of what would go into this," he added.

That uncertainty included setting any age limit for the warning at this point in time. "The committee discussed the issue [of an age cut-off] but did not vote on it, Pine said. "Opinions were mixed."

Pine added that the FDA and the panel members agreed that "the level of detail in the data was such that it was not possible to give a precise age at which it was particularly concerning in terms of the association with suicidal thoughts or behavior, or not concerning."

Discussion on a more precise wording is set for the near future, the panelists said.

"The committee generally endorsed what we [FDA] had done and agreed with our interpretation of data," added Dr. Thomas Laughren, director of the division of psychiatry products at the FDA's Center for Drug Evaluation and Research. "The general agreement is that there does appear to be an age-related causal link to suicidality, but there was a lot of discussion about the need to do further research to find the basis for the apparent relationship.

There was also a good deal of concern on the effect black boxes may have had on discouraging appropriate prescribing."

Outside experts echoed that concern.

Although the recommendations were based on fears that taking antidepressants could increase the risk of suicide ideation, many psychiatrists worry that the "black-box warning" will backfire -- resulting in more suicides as patients shy away from depression treatment.

Expanding the warning "will increase the completed suicide rates," said Robert D. Gibbons, director of the Center for Health Statistics and a professor of biostatistics and psychiatry at the University of Illinois in Chicago. His research found an increase in completed suicides in children after the black-box warning was added.

Gibbons also pointed out that current black box warnings, although intended only for children and adolescents, have caused a decline in SSRI prescription rates for adults as well as in those for the younger age group.

Others agreed. "The existing black box warnings for children and adolescents has clearly steered people away from both prescribing these drugs, and from patients and families being willing to accept treatment," said Dr. Norman Sussman, a professor of psychiatry at New York University School of Medicine in New York City. "It'll probably happen in terms of a global effect with adults as well."

"The FDA's decision to implement black box warnings for all antidepressants in pediatrics has led to a precipitous decline in prescriptions," added Dr. David Fassler, a child and adolescent psychiatrist in Vermont and a trustee of the American Psychiatric Association. "I'm concerned that there are many young people with depression who are not currently receiving appropriate and effective treatment. My own hope is that the FDA exercises caution before deciding to extend this warning to the use of these medications with adults."

The FDA is not obligated to follow the recommendations of its advisory committees but usually does.

SSRIs, the class of antidepressants that includes Prozac (fluoxetine) and Paxil (paroxetine), have been the subject of intense controversy in recent years.

In October 2004, the FDA directed manufacturers of SSRIs to revise their labeling to include a black-box warning that alerts health-care providers to an increased risk of suicide and suicidal thoughts in children and teens.

In July 2005, the FDA issued a public health advisory that raised the possibility that the risk of suicidality also applied to adults taking SSRIs, after several studies pointed to that possibility.
More recently, however, a major new study found that SSRIs have actually saved thousands of lives by preventing suicides since they were introduced in 1988.

The FDA advisory panel considered an FDA-conducted meta-analysis of 372 studies involving more than 100,000 people taking antidepressants. That analysis found that the drugs might increase the short-term risk of suicidal thoughts and behavior among adults aged 18 to 24.

According to an Associated Press report, the FDA estimated the increased risk to be at as many as 14 additional cases of suicidal thoughts or behavior for every 1,000 children treated with antidepressants. For adults aged 18 to 24, there could be four additional cases per 1,000 people treated.

Some experts, including Gibbons, had questions about the integrity of the data in the review.
"The studies they're basing this on screened out people who were suicidal to begin," Sussman said. "You can't make any kind of inference on how many lives were saved because the depression was treated. They're saying the risk of committing suicide outweighs the benefit [from treatment], and that isn't necessarily accurate."

The panel meeting unleashed a veritable hailstorm of reactions and testimony from families of suicide victims, depressed patients, psychiatrists and activists, even before any recommendation was issued.

According to the AP, Ellen Hanson testified that her husband, Scott, hanged himself in 2004 shortly after starting on Paxil. The couple did not know there might be a suicide risk associated with the drug. "I know he didn't want to be dead," she told the panel. "Even two years later, I feel his death was an accident -- a medical accident."

But even suicidal thinking associated with the drugs is rare, Sussman said. "In reality, it's such a small percentage," he said. "It gets in the media and creates the false impression that these are dangerous drugs."

And one panel member worried that depressed individuals might back away from helpful medications. The solution may lie in close follow-up for all patients, said Dr. Robert Temple, director of the FDA's Office of Drug Evaluation, Center for Drug Evaluation and Research.

"We have a concern that people are not getting treated," Temple said at the Wednesday press briefing. "We think everybody at the time they are started on antidepressants ought to be observed closely because it's a dangerous time. This applies to everybody, even those over 65."
"We persist in believing that that's an important enough message that we don't want to discourage appropriate use," he added. "But we still think it needs an appropriate warning."

Rare Gene Brings Life Without Pain

(HealthDay News) -- A rare genetic mutation makes it impossible for an otherwise healthy carrier to feel pain, researchers report.

The mutation -- discovered in three related families in northern Pakistan -- advances the understanding of the genetic basis of pain and might lead to new pain treatments, the international team said.

The findings are published in the Dec. 14 issue of Nature.

"Occasionally, rare genetic diseases can teach us lessons about more common disorders," explained Dr. Stephen G. Waxman, chairman of neurology and director of the Neuroscience and Regeneration Research Center at Yale University and author of an accompanying journal commentary.

Individuals with the rare genetic mutation cannot feel any pain but have otherwise completely normal sensory functions.

One case the researchers studied involved a 10-year-old Pakistani street performer born with the genetic mutation. In his act, he placed knives through his arms and walked on burning coals.

The boy later died on his 14th birthday from injuries he suffered after jumping off a roof.

The Pakistani discovery "really illustrates the pivotal effect that one molecule can have [on pain]," Waxman said. "We do not yet have a cure for common pain, but we hope that there will be a totally new class of drugs to treat pain."

"This paper shows that rare diseases can still be of great importance because of the insights they give into biological and developmental processes," the study's lead researcher, Dr. Geoffrey Woods, from the Department of Medical Genetics and the University of Cambridge Institute for Medical Research, U.K., said in a prepared statement.

Among the three related families the researchers studied, six relatives had not felt pain at any time in their lives. Neurological examinations found no evidence of motor or sensory disease. The affected individuals could still feel a number of sensations, including touch, warm and cold temperature, tickle and pressure.

Pain is not all bad, experts say. In fact, it is a survival mechanism that enables people to be aware of and minimize damage to tissues. The researchers noted that all of the carriers of the mutant gene showed signs of a variety of injuries -- including wounds to their lips and/or tongues from biting themselves when young.

Woods's group found that a mutation in a gene called SCN9A causes a loss of function in the "voltage-gated sodium channel" it encodes. Sodium channels are proteins that excite nerve cells. Though the precise function of this channel is unclear, it appears to play a part in exciting sensory neurons.

"The work of Geoff Woods and his team has provided us with an exciting new target for pain-killing drugs," coauthor Dr. John Wood, from University College London, said in a prepared statement. "Potentially, this is as important as the identification of the morphine receptors. It is fascinating that this same gene, when mutated to encode a hyperactive channel, has also been found to contribute to ongoing pain in some heritable human disorders," he said.

Since people with mutations in SCN9A are otherwise healthy, the researchers hope that compounds that block sodium channel functioning could become effective analgesics.

But Waxman stressed that the Pakistani mutation is not the only one associated with pain.

"We came across a rare genetic disorder called erythromelalgia, also called 'man on fire syndrome,' " Waxman said. "These are people who have pain to any mild warmth. Going into a warm room, putting on a pair of socks, exercise -- that triggers severe burning pain. They describe it as if hot lava were poured onto their hands and feet."

It turns out that families that have this syndrome have a mutation in one of the sodium channels, Waxman said. "This mutation causes this channel to have enhanced function," he said.

Knowing the basis for the problem, it might be possible to develop a drug to treat it, Waxman said. "One could develop drugs that turn down or turn off the channel," he said.

Another expert says that this study confirms the genetic link to how we experience pain.
"There is tremendous variability based on genotype in response to painful stimuli," said Dr. Rollin Gallagher, the director of pain management at the Philadelphia VA Medical Center and a professor of psychiatry and anesthesiology at the University of Pennsylvania School of Medicine.

This study substantiates what is seen clinically, Gallagher said. "It shows the difference in individuals in their opioid receptors. This also gives us a direction for research and suggests that treatments can be found to treat sodium channel defects," he said.

Weight Training Helps Women's Bones

(HealthDay News) -- Long-term weight training may help women's bone and metabolic health by promoting increased production of growth hormone, new research suggests.

Growth hormone, produced in the pituitary, plays an important role in bone and muscle development in women, while men rely more on testosterone, according to background information in the study.

Researchers at the University of Connecticut and elsewhere conducted the study, which is published in the December issue of the American Journal of Physiology-Endocrinology and Metabolism.

Growth hormone also plays a role in fighting tissue breakdown, improving metabolic function and staving off stress fractures.

In this study, researchers compared different weight-training regimens and different testing methods and concluded that the role of growth hormone in women's muscle development may be more complicated than previously believed.

"We found that growth hormone was responsive to moderate and heavy exercise regimens having 3 to 12 repetitions with varying weight loading," principal author William J. Kraemer, of the University of Connecticut, said in a prepared statement.

"Women need to have heavy loading cycle or workout in their resistance training routines, as it helps to build muscle and bone," he said.

'Double Diabetes' a New Threat

(HealthDay News) -- Despite the flurry of public service campaigns and education efforts, the diabetes epidemic in the United States continues to escalate out of control.

An estimated 20.8 million Americans -- or 7 percent of the population -- are now believed to be diabetic. Of those, 6.2 million people have the disease but don't know it. And that doesn't include the 41million people with pre-diabetes, a condition in which blood-glucose levels are higher than normal but not high enough to be diagnosed as type 2 diabetes.

In fact, the epidemic has become so pervasive that doctors are now finding patients who suffer from both type 1 and type 2 diabetes -- a phenomenon known as "double diabetes" or "hybrid diabetes."

"It's mostly people who have a type 1 diabetes who become overweight and show the profile of a type 2, with obesity and hypertension," said Dr. Stewart Weiss, an assistant clinical professor of medicine at New York University School of Medicine in New York City.

Doctors and health-care experts are urging people to take the steps necessary -- basically proper nutrition and plenty of exercise -- to avoid joining the ranks of those already diagnosed with the disease.

Type 1 diabetes is caused by the body's inability to produce insulin, the hormone that ushers blood sugar -- called glucose -- to cells for energy. An estimated 5 percent to 10 percent of Americans with diabetes have type 1 disease. Type 2 diabetes results from insulin resistance -- the body's inability to properly use the hormone. Most Americans diagnosed with diabetes have type 2 diabetes, and excess weight and lack of exercise are big contributors to this form of the disease.

But, doctors are now seeing strong indications that double diabetes is a growing phenomenon. For instance, recent studies suggest that as many as 30 percent of newly diagnosed diabetes cases among children involve youngsters with both type 1 and type 2 diabetes.

Generally, double-diabetes sufferers will often look as though they have the more common type 2 version because they're overweight. But subsequent blood tests reveal they also have type 1 disease.

Double diabetes takes the suffering caused by the disease a step further, and complicates efforts to treat it.

Type 1 diabetics normally have to take daily injections of insulin to remain healthy, while type 2 diabetics require different medication and regular monitoring of their blood sugar. Doctors now are researching how to juggle treating both types of diabetes in the same patient, Weiss said.

"We have all sorts of medications that address different problems for different types of diabetes," Weiss said. "The question with double diabetes becomes, when can we use the different types of medications and what would be appropriate for different patients?"

Weiss suspects that double diabetes might be caused, in part, by type 1 diabetics who are taking insulin but haven't made the other lifestyle changes necessary to deal with the disease.

"One of the consequences of proper insulin use is weight gain," he said. "Often, patients who have not had a good understanding of how to eat are taking the insulin to cover what they normally eat."

The national trend toward unhealthy weight gain has spurred both the diabetes epidemic and this newer, more complex form of the disease, agreed Dr. Francine Kaufman, past president of the American Diabetes Association and head of the Division of Endocrinology and Metabolism at Childrens Hospital Los Angeles.

"It's mirroring the obesity epidemic," Kaufman said.

Diabetes can cause a number of complications that range from life-damaging to life-threatening. They include heart disease and stroke, high blood pressure, blindness, diseases of the nervous system, lower-limb amputations, dental disease and complications during pregnancy.

The best way to avoid these complications is to avoid contracting diabetes. A recent study conclusively showed that people on the verge of contracting type 2 diabetes prevented its onset through changes to their diet and increased exercise, according to the American Diabetes Association.

The federal Diabetes Prevention Program study found that diet and exercise actually worked better than some medications in delaying the development of diabetes, according to the association. Just 30 minutes a day of moderate physical activity, coupled with a five percent to 10 percent reduction in body weight, produced a 58 percent reduction in diabetes.

"People need to maintain a healthy weight and a healthy lifestyle," Kaufman said.

Weiss recommends improving diet by eating more vegetables, fewer starches and more lean meat and fish. "I like to say there's no medication that can overcome a bad diet," he said.

He also said people should not only eat healthier, but they should try to eat less, too.

"The problem really is overeating in general," he said. "The portion size put before us is very large, so portion control is the single most important thing. You've got to know when to say when."

(HealthDay News) -- If you're considering running a marathon, you'll need a carefully planned training schedule to avoid injury. Here are suggestions on how to prepare for the race, courtesy of the Cleveland Clinic:


  • Start training at least 18 weeks before the marathon. Ideally, you should have been running regularly for at least a year before the actual race.

  • Take time off every week from running to let your body rest and prevent injury. You should not run every day.
  • Follow up a very strenuous run with an easier day. By alternating strenuous and lighter workouts, you'll help prevent injury.
  • Three weeks before the marathon should be your last long run. Your body will need that time before the marathon to rest.

Health Tip: Beware of Poisonous Holiday Plants

(HealthDay News) -- Poinsettia plants aren't as poisonous to people and pets as was once believed, the Minnesota Poison Control Center says. But poinsettias do have a mild irritant that can cause nausea, vomiting and diarrhea, the center warns.
Other popular holiday plants, however, are more dangerous if ingested. The poison center offers this list:

  • Holly -- this plant's bright red berries are highly toxic and can be fatal if ingested, even in small amounts.
  • Mistletoe -- berries, leaves and stems of this plant are all poisonous.
  • Jerusalem cherry -- the bright berries and all other parts of this plant are poisonous.
  • Bittersweet -- berries, leaves and stems are all toxic.
